Who can I nominate?
You can nominate an INDIVIDUAL or a PROVIDER in one of three award categories: INSPIRATION, INNOVATION or ADVOCACY.

What’s the difference between an individual or provider?

  • INDIVIDUAL: Any one person – a member, family or community member or a single practitioner. Your nomination should highlight that individual’s personal effort, approach or initiative in the specific category in which you’re nominating them.
     
  • PROVIDER: An organization, practice, facility, health home, program or team. Your nomination should highlight the provider’s effort, approach or initiative in the specific category in which you’re nominating them.

Can I nominate more than one individual or provider for a RISE Award?
Yes. You can nominate as many individuals or providers as you’d like. However, you can only nominate a specific individual or provider once and only in one award category.

Can I submit the same nomination for an individual or provider in more than one category?
No. You can only nominate the same individual or provider once and in one award category. Make sure your entry is specific and tailored to the category you’ve selected for the nomination.

Can I nominate the same individual or provider in all three categories?
No. You should pick the category that best represents or captures the effort, approach or initiative of the individual or provider you’re nominating. 

If I nominate a team in the provider category, will each team member receive a RISE Award?
A provider selected as an honoree will receive a single RISE Award statue displaying the name of the provider (organization, practice, facility, health home, program or team) as it appears in the nomination form.

 What information should I include in my nomination?

  • Specific, detailed examples that answer these types of questions:
    • Why are you nominating this individual or provider?
    • What did they do?
    • How did they do it?
    • How did it empower members or encourage resiliency?
    • What happened as a result of their actions?
    • How did their service benefit you or others?
    • How are others replicating or building on their innovation?

Can I get an extension on the nomination deadline?
To ensure that everyone has the same opportunities, we will not make exceptions for late entries. Start thinking early about who you’d like to nominate. Do your best to submit your nomination well before the deadline in case you experience technical issues or other unexpected delays.

Can I submit supporting materials with my nomination?
No. Nominations are limited to the space allocated in the nomination form to help ensure all nominations are evaluated based only on their content, including specific, detailed examples of the individual or provider actions and the impact of their actions.

Can I nominate a Mercy Care employee for a RISE Award?
No. Mercy Care employees aren't eligible for a RISE Award.
